All Athletes regardless of sport must upload a copy of a physical to ParentVue as well as complete the cardiac arrest form in order to register for the Golf program. Athletes that have already completed these for a fall or winter sport do not have to repeat this as the physical and cardiac arrest form are valid for one year.
